Paul
Stressed about how his fellow peers will accept him in his new position of leadership, Paul offers to do the station's less than desirable chores to please his friends, and offers to do more jobs at rescues they respond to to show the crew he's a competent leader. When he injures himself, he has to take a step back from a rescue, and later, when he sees none of the less desirable chores have been done, he inserts his authority and orders his fellow crew to go take care of those chores.

Tommy and Trevor
Tommy and Trevor host Cassandra for dinner at Tommy's place. Right from the get go, Cassandra is a little bit stand-offish with Tommy, and throughout the dinner, starts feeling sick. Cassandra accuses Tommy of poisoning her food, and then promptly throws up. Tommy later confers with Nancy and TK, and they come up with the possibility that Cassandra is sabotaging Tommy to appear as a bad mother so that Trevor won't marry her. Deciding to do surveillance, Tommy sets up a baby monitor camera in her house, and later gets a call from her daughter's school about a vape being found in her backpack. Thinking Cassandra planted it, Tommy confronts her, but is interrupted when Melody explains she did everything to sabotage Tommy because she wanted to go back and live with her mother again. Knowing she couldn't be in a long-distance relationship with Trevor, Tommy breaks things up with him, knowing he would want to be near his daughter too.
126 Rescues
The 126 respond to two rescues this episode. The first is at a rat-infested hoarder's house, in which an elderly lady has taken a fall, and her watch alerted 911 she had fell. On the way to the ambulance, the lady asked if someone could go and find her purse. Offering to go, Paul heads into the house, but drops through the rotted floor, finding the lady's dead cat in the process.
During the second rescue, Tommy, Nancy and TK respond to a highrise for a pregnant woman, Opal, trying to have an at-home birth, but due to complications they decide to transport her to the hospital. On the way down, the elevator loses power and they get stuck between floors, having to wait for the 126 firefigthers to come to their rescue.

While waiting, Opal goes into labor, having her baby in the elevator. The joy of her giving birth is shortlived though, when Opal starts bleeding from her eyes and nose and goes unresponsive, forcing another fire station to be called for help to deliver O- blood. When the blood is delivered, the 126 fire crew begin the process of breaking the wall to get access to the elevator. The paramedic trio is able to revive Opal with CPR and the O- blood given to her in time.
Thoughts
I enjoyed seeing another episode that had a focus on the paramedic trio, as Ronen Rubinstein, who plays TK, is my favourite actor, so I always love the paramedic scenes because I get to see him. I really love how he's always so gentle and caring of the patients he's caring for.
I loved seeing Paul take authority of his role at the end of the episode, and can't wait to continue see him shine as lieutenant.
Next Episode
Episode five, 'Thunderstruck,' airs on October 21.
5x05 synopsis: A horse barrels into a brewery with an unconscious rider on its back; a bus full of tourists all experience seizures; Judd introduces Owen to Wyatt's mother; Carlos' investigation into his father's murder becomes all-consuming.
